Are you a new patient?

• For OHIP consultations, you need a referral from a doctor.
• For non-OHIP-covered treatments (for example, removal of moles, cysts, skin tags, spider veins, age spots and most warts), you do not require a referral from a doctor and can make an appointment with us directly.
• For clinical trials, no referral is required. You can contact us directly.
• We ask all new patients to complete a questionnaire. We can handle your visit more efficiently if you complete this form in advance and bring it with you to your first appointment:

Are you an existing patient?

You can make an appointment directly with us if your last visit was less than one year ago.
